Guidance/Student Services

To request official transcripts:

• Fill out a Transcript Request Form
• Call the guidance department directly
• Once you have submitted your transcript request, please e-mail an uploaded image of your photo ID to Michelle Vitale  to allow for your request to be processed.

If you are requesting special education/504 documents, please call the Office of Exceptional Education and Student Services at 716-626-8061.

For working papers:

• You must have a proof of physical on file with South High School within the past year or you need to provide an updated physical

• Please review the following rights and know your rights

  • NYS Child Labor Law Card for Workers (P886)

  • NYS Child Labor Law Card for Employers (P887) 

• Please sign and return the application to guidance
